National Walk for Values 2009

Register

You can register for this year’s Walk in Perth by:

  1. Filling in the registration form below
  2. Downloading a copy here and bringing the completed form with you to the registration desk on the day of the Walk.

In order to register, you must agree to the terms of the Consent to Volunteer and Waiver of Liability stated below and on the back of the Registration form. A parent or a legal guardian must sign for anyone who is below the age of 18. Children under the age of 12 must be accompanied by a parent/guardian. Fields marked with * are compulsory.

Consent to Volunteer and Waiver of Liability

By providing the information below for registration, you acknowledge that you have read, fully understand and agree to the Consent and Waiver given below:

"I have read the guidelines herein in relation to the National Walk for Values (The Walk) and I wish to be a volunteer to take part in the Walk. I understand that participation is purely voluntary and there is no monetary remuneration. I have sought medical advice as to my suitability to take part in the Walk. I undertake to take adequate precaution for my safety and that of the other participants. I will wear clothes appropriate to the weather conditions and carry sufficient food and water to ensure my basic needs. I hereby release, indemnify and hold harmless the Organisers, participants and other participating Organisations in respect of any loss, damage or injury including death whatsoever and howsoever arising directly or indirectly from my participation in the Walk."

Guardian Form of Consent and Waiver

"I , who resides at this address: as parent/guardian of of years of age

have read the guidelines and Information Package in relation to the National Walk for Values (The Walk). I understand that the participation is purely voluntary and there is no monetary remuneration. I have sought medical advice as to the suitability of my child/ward to take part in the Walk.I undertake to take adequate precaution for my child's/ward's safety and that of the other participants. I shall further ensure that my child/ward will wear clothes appropriate to the weather conditions and carry sufficient food and water to ensure his/her basic needs. I hereby give consent to his/her participation in the Walk. I release, indemnify and hold harmless the Organisers, participants and other participating Organisations in respect of any loss, damage or injury including death whatsoever and howsoever arising directly or indirectly by his/her participation in the Walk. I further take the responsibility for the supervision and care of the participant abovenamed."

General Terms

  1. I understand that bicycles, skateboards, baby joggers, roller skates or blades, animals, and radio/cassette/CD headsets are not allowed in the Event.
  2. I agree to be bound by all Rules and Regulations submitted by the organisers and local Rules and Regulations in effect within the City and the State, at all times.
  3. I agree to comply with all instructions given by the Organisers and Walk Marshalls during the walk itself, and that I will walk only on the specified route of the walk.
  4. I understand fully that a parent or guardian must sign the waiver for any participant under the age of 18 and that an adult must accompany, at all times, any children under the age of 12.
  5. I confirm that all the information I have provided is correct.
  6. If I am requested to, or do want to, display a value placard or sign, I understand that the said placard or sign needs to be cleared by the organisers of this event, to allow me to carry it in this walk and/or in the vicinity of this event.
